Abstract: 

Introduction: The intrauterine DEVICE ((IUD)) has been used for years for contraception and is used so far in more than 100 million women. (IUD) migration into adjacent viscera has been reported, but rarely migrates to the bladder. (IUD) migration into the bladder can cause stone formation. Many patients are asymptomatic and may have only irritative and obstructive urinary complaints. In this study, a patient is introduced that 10 years after insertion of (IUD) referred with hematuria and a stone on (IUD) migrated to bladder was found and improved with endoscopic treatment.Case Report: A 45-year-old woman with recurrent urinary tract infections and a history of gross hematuria referred to the clinic. Patients had a history of (IUD) insertion about 10 years ago. Abdominal and pelvic sonography revealed a 2´4 cm stone in urinary bladder. The patient undergone cystoscopy and a large stone was observed in the bladder that was quite nearby (IUD).Endoscopic lithoclast broke stone and (IUD) was removed.Conclusion: Presence of irritative urinary symptoms and recurrent urinary tract infections in the women who have a history of (IUD) insertion, migration of the DEVICE to the urinary bladder should be considered. In these cases, Ultrasonography and cystoscopy can help to the diagnosis.

Abstract: 

Background & Aim: Intrauterine DEVICE and depo-medroxyprogesterone acetate are among the most effective reversible contraception in the world, still few consensuses exist about sexual and mood changes of these two conventional methods. The present study has compared the sexual satisfaction and depression level in these two methods. Methods & Materials: A cross-sectional study was conducted between August 2017 and January 2018, in 300 married women aged 19-50 year, in Tehran, Iran. One hundred and fifty (IUD) users and 150 DMPA consumers, 12-18 months after starting the current method, recruited to the study from 34 health centers of Tehran University of Medical Sciences by convenience sampling. Two standard questionnaires of Index of sexual satisfaction (ISS) and Patient Health Questionnaire (PHQ) were used for assessing sexual satisfaction and depression, respectively. Chi-square and independent t-test were used for the homogeneity of the two groups. The linear logistic regression analysis was conducted to estimate the strength of associations. Results: There was not any significant difference between two groups in demographic characteristics such as age, marriage duration, education level, number of children and breastfeeding condition (P>0. 05). Significantly higher sexual satisfaction (P<0. 001) and lower depression level (P<0. 001) reported in (IUD) users than DMPA consumers in the t-test. A multivariate regression confirmed that sexual satisfaction increased 6. 4 scores in the (IUD) group in comparison to DMPA users (P= 0. 003, B=-6. 4), and with increase 1 year to duration of the marriage, sexual satisfaction increase 0. 33 score. For depression, the only significant variable was the contraceptive methods, and depression level increase 1. 24 scores in DMPA users than (IUD) consumers. (P=0. 006, B=-1. 24). Although in univariate analysis, some variables showed effects on sexual satisfaction and depression, multivariate regression results did not confirm any significant relationships. Conclusion: Both sexual satisfaction and mood level was higher among (IUD) users. This paper reconfirmed that (IUD) is a preferable method in women who are eligible to use both methods.

Abstract: 

Objective: Despite Intra Uterine DEVICE ((IUD)) has the advantage of providing long-term contraception and does not require daily attention, only %8 of Iranian women use it. This study was conducted to enhance understanding of why women in the Isfahan appear reluctant to consider (IUD) as a contraceptive method.Materials and methods: It was a qualitative study that was done in Isfahan in May 2009. Five urban health centers of Isfahan city were selected randomly. In each center by purposive opportunistically sampling, interview was done with referring women of reproductive age that had never used (IUD) without any contraindication by one interviewer. One to one semi-structured interviews were continued until a level of information saturation was reached and no new themes were emerging. Each woman's believe about (IUD) and her reasons for not using (IUD) as a contraceptive method was asked by interviewer. Also all participants were asked about the official (leaflets, health care professionals, books and school) and unofficial (personal narratives recounted by friends and relatives) sources of their information. Eleven interviews were conducted in total. key words in ideas of each woman were identified to develop major misconceptions about (IUD) use.Results: Fear of side effects of (IUD), religious believes, anxieties related to fitting of an (IUD), make difficulty in intercourse and damage to fetus were the major misconceptions about (IUD) use. Religious believes were the most impediment factor for (IUD) use.Conclusion: All of the items identified mirrored those found in other studies except the prominent worries about religious aspects of using (IUD) that are new and need wider exploration.

Abstract: 

Intrauterine DEVICEs ((IUD)s) provide effective and reversible contraception, but rare pregnancies can lead to complications like miscarriage and preterm labor. Timely removal during the first trimester is advised to reduce risks. This case reports a 17-year-old female who presented with abdominal pain and vaginal bleeding after 16 months of (IUD) use and a 2.5-month missed period. Despite confirming a 13-week gestation via transvaginal ultrasound, she opted to continue the pregnancy without (IUD) removal. However, she spontaneously expelled both the fetus and (IUD) after two hours of observation, with minimal bleeding and no complications. This case highlights the risks associated with retained (IUD)s during pregnancy, emphasizing the importance of post-insertion follow-ups and (IUD) positioning checks. If pregnancy occurs, early removal is recommended to prevent adverse outcomes.

Abstract: 

Although intrauterine DEVICEs ((IUD)s) are known for their low failure rate in pregnancy prevention, potential risks associated with their use include uterine perforation and migration through the abdomen. In this particular case, the patient experienced simultaneous (IUD) failure and perforation, with the DEVICE becoming embedded in an omentum. A 28-year-old woman who was 39 weeks plus one day gestation presented for caesarean section. During the caesarean section, it was discovered that the (IUD) had entered the omentum through a hole in the posterior part of the uterus. The result of the birth was a live boy. (IUD) perforation is most commonly observed during the insertion procedure,however, it may rarely occur at a later stage. (IUD) perforations are frequently asymptomatic and remain undetected until follow-up assessments are conducted or clinical manifestations become apparent. The presence of gastrointestinal symptoms is commonly observed in cases with intraperitoneal migration of the (IUD). Although the occurrence of abdominal pain, diarrhoea, and fever with a missing (IUD) are indicative, they may not always be present. Nonspecific signs and symptoms lead to misdiagnosis and the consequent delay in initiating appropriate treatment. In the present case, the co-occurrence of (IUD) embedded in the omentum and pregnancy posed a challenge due to the DEVICE’, s rapid and imprecise shift, which complicated its accurate localisation. In such scenarios, ultrasonographic guidance can serve as a valuable tool to enhance accuracy and decrease adverse outcomes.
